"Erespal" is a modern medicine for the treatment of many pathological processes that occur accompanied by cough in people of different age groups. It differs from other antitussive and mucolytic drugs in the combined therapeutic effect - bronchodilating, anti-inflammatory, antispasmodic and anti-allergic. "Erespal" is not an antibiotic and does not have side effects characteristic of this group of drugs.

The composition of the drug for cough
The active therapeutic basis of the drug is fenspiride hydrochloride.
The medicine is produced by companies in France and Russia in two dosage forms:
- White convex round tablets in an enteric-soluble membrane containing 80 mg of a therapeutic substance. The pharmacy pack contains 2 aluminum-plastic blisters with 15 tablets and medical instructions.
- A syrup in the form of a brownish-yellow liquid, in which a precipitate may form (eliminated after shaking). In 1 milliliter of syrup is 2 mg of fenspiride. It is bottled in plastic brown-transparent bottles (150, 250 ml).
Inactive components:
- in tablets: stabilizer of the consistency of calcium hydrophosphate, hypromellose, polyvinylpyrrolidone with viscosity K30 (as a binder and a prolonged action of the drug), silicon dioxide (as an emulsifier), magnesium stearate (as a filler), food coloring titanium dioxide,water-retaining agent glycerol and macrogol 6000 as a shaper.
- in syrup: sucrose, natural flavoring (sunflower honey), as preservatives - potassium sorbate, propyl parahydroxy and methyl parahydroxybenzoate, natural licorice extract, glycerol as a thickener, vanilla tincture, food color yellow CFC, saccharin, water, ethanol (no more 0.28 - 0.29 mg in 100 ml of solution).
Pharmacological properties and pharmacokinetics
Fenspiride has multiple therapeutic effects:
- effectively reduces inflammation, suppressing the mechanism of its occurrence - reduces the production of various hormone-like protein substances involved in the development of the inflammatory reaction;
- reduces the frequency and severity of coughing attacks, without affecting the center of the brain;
- reduces the viscosity of sputum and contributes to its easy discharge, quickly converting a dry obsessive cough with thick, sparse sputum into productive wet;
- expands the bronchi, preventing the development of spasm and obstruction of the bronchi;
- reduces the severity of allergies, slowing down the release of histamine, which regulates the body's response in the form of allergic manifestations;
- inhibits the development of swelling of the mucous membrane of the throat and respiratory tract.
The healing component is actively absorbed into the blood. The highest plasma fenspiride content after drug administration is observed after approximately 2 - 2.5 hours. It is removed from the body mainly by the kidneys and only in the amount of 10% - along with feces.
Why prescribe Erespal syrup and tablets
"Erespal" is prescribed as a single drug or as part of a combination treatment to eliminate cough, relieve spasm of the bronchi and swelling of the nasopharynx, facilitate breathing.
Used in the following painful conditions:
- bronchitis, tracheobronchitis;
- bronchial asthma (complicated, isolated);
- cough, swelling of the nasal mucosa and throat, hoarseness with rhinopharyngitis, laryngotracheitis, rhinotracheobronchitis and other infectious and respiratory infections of the throat and respiratory tract;
- cough and inflammation on the mucous membrane of the throat, pharynx, trachea against the background of whooping cough, flu, measles;
- allergic rhinitis and other signs of allergy from the respiratory organs and oropharynx;
- sinusitis, otitis media of various nature, including allergic forms.
Instructions for use and dosage for children and adults
The absorption of the drug is higher if it is taken before meals, but the medicine in the form of a syrup is allowed to be added to food for babies from 2 years old.
Erespal tablets 80 mg
The medication in tablets is intended for adults and adolescents from 14 to 15 years. Take "Erespal" should be 2 times a day for 1 tablet. In severe conditions, the frequency of admission can be increased to 3 tablets per day. The greatest amount of fenspiride that a patient can get (from 14 years old) per day is 240 mg (3 tablets).
The duration of therapy is determined by the doctor. Since the medication is not an antibiotic, in mild conditions, 3 to 4 days of admission are sufficient to achieve a noticeable therapeutic effect.
Children under 14 years of age are advised to give the medicine in the form of a syrup to avoid overdosesince the maximum pediatric dose is limited to 120 milligrams of fenspiride per day.
Erespal in the form of cough syrup
For children 2 to 14 years old, the daily amount of syrup is prescribed at the rate of 4 mg per 1 kilogram of weight (per day). To easily measure the desired dose, use a measuring cup.
Babies from 2 years old (if the body weight is not more than 10 kg) are given 2 to 4 teaspoons of the solution (10 to 20 ml) per day. 1 teaspoon (5 ml) contains 10 mg of fenspiride.
Based on the fact that 1 ml contains 2 mg of the therapeutic component, and the amount of medication for a child per day is calculated at a rate of 4 mg per 1 kg, you can easily calculate the exact dose by weight of the child. The resulting volume is divided into 2 to 4 times.
Children from 2 to 14 years old (weighing more than 10 kg) per day will need from 30 to 60 ml, that is, no more than 120 mg of fenspiride. A tablespoon (15 ml) contains 30 mg of the active substance.
But it is best to accurately calculate the dose, especially for children under 7 - 10 years. For example, a child weighs 15 kg, which means that in a day he can receive no more than 15x4 = 60 mg of fenspiride (30 ml of syrup or 2 tablespoons). This dose is divided into 2 to 6 doses.
Adult patients (from 15 years old) can drink Erespal syrup from 3 to 6 tablespoons of the medicinal solution per day.
Shake the syrup before taking it so that the drug sediment evenly spreads in the bottle.
During pregnancy and lactation
It is not recommended to take Erespal to patients awaiting the birth of a baby, since clinical data on the effect of fenspiride on the fetus and pregnancy are limited.
If a woman is pregnant, when she is being treated with Erespal, this is not considered a reason for her termination. But since in animal studies there have been cases of the appearance of a “cleft palate” in embryos, therapy should be discontinued.
Information on the amount of fenspiride that passes into breast milk is not available, therefore, in order to prevent drug poisoning of the baby, the nursing mother should transfer the baby to artificial feeding for the period of drug therapy.
Drug interaction
The effects of the combined use of fenspiride with other pharmacological substances are unknown, since the interaction of Erespal with other medicines has not been studied.
However, it should be borne in mind that the sedative (sedative) effect may intensify and marked drowsiness may occur with the parallel use of a medication with drugs that have a sedative effect, antiallergic drugs, as well as with alcohol.
Contraindications, side effects and overdose
The medicine "Erespal" is contraindicated:
- adolescents under 14 years old (in tablet form) and children under 2 years old (in syrup form);
- with special intolerance to fenspiride and any of the components of the drug.
Due to the introduction of sucrose into the syrup, the medicine is very carefully prescribed:
- patients with diabetes;
- with a syndrome of malabsorption of glucose and galactose;
- patients receiving hypoglycemic drugs;
- people who do not tolerate fructose, or suffering from a deficiency of sucrose-isomaltase (dose adjustment possible downward).
For reference: in a teaspoon (5 ml) contains 3 grams of sucrose, in the dining room (15 ml) - 9 grams.
Adverse reactions to taking fenspiride develop very rarely, and, as a rule, when exceeding the dosage or uncontrolled long-term use (especially in childhood). Usually eliminated by dose reduction.
May be observed:
- insignificant increase in heart rate;
- nausea, soreness in the pit of stomach, loose stools;
- drowsiness, dizziness, increased fatigue;
- in people with drug and food allergies: redness of the skin, itchy rash, blisters, in isolated cases (not registered) - swelling of the eyelids, lips, tongue, throat.
Signs of an overdose can appear with long-term use of the drug in high doses.
You should immediately contact the Ambulance service if overdose symptoms are noted in the form of:
- abnormal daytime drowsiness, overexcitation, dizziness;
- severe tachycardia (more than 100 - 120 beats per second);
- nausea with bouts of vomiting, diarrhea;
- acute allergic reactions, especially laryngeal edema.
In these cases, medication is immediately discontinued and absorbents are taken if possible (preferably, Polysorb, as the most effective).
In the hospital, gastric lavage, detoxification and anti-allergic therapy are carried out, cardiac functions are monitored on electrocardiography.
Special instructions:
- Erespal does not replace antibiotics, therefore, when signs of severe inflammation appear - fever, wheezing, hard cough - they immediately call a doctor to prescribe antibiotics;
- the composition of the drug solution includes hydroxybenzoates, so treatment with syrup can cause acute and late allergic reactions;
- in the early days of therapy or when combining "Erespal" with alcohol, a state of drowsiness often appears, which requires special attention when driving vehicles and working with complex and dangerous equipment.
